Background : Postpartum cerebral angiopathy (PCA) is rare. Previously reported cases suggest that a use of vasoconstricting agent play a role in the pathogenesis of PCA. We report a patient of PCA without use of any vasoconstricters. Case report : A 27-year-old woman presented with severe headache, vomiting and left-sided weakness five days after her first delivery. Brain CT showed cerebral hemorrhage in the left temporal area. Cerebral angiography revealed diffuse stenosis in the bilateral middle cerebral, vertebral and basilar arteries. After intravenous dexamethasone therapy, she gradually improved. Two months later, a repeated cerebral angiography revealed normalized cerebral vessels. Conclusion : Our patient suggests that PCA may occur in the absence of history of sympathomimetic drug use. The underlying pathogenic mechanism of PCA remains obscure.
