Two Korean men (ages, 51 and 54 years), both life-long residents of Korea with no history of foreign travel, blood transfusion or injection drug use, were diagnosed with adult T-cell leukemia/lymphoma (ATLL) in 1994. Human T-cell lymphotropic virus type I (HTLV-I) infection was confirmed by serology and polymerase chain reaction (PCR). Despite initial improvement following intensive chemotherapy, both patients died. To determine the genetic diversity and molecular phylogeny of HTLV-I in Korea, we enzymatically amplified, then directly sequenced selected regions of the gag, pol, env and pX genes of HTLV-I proviral DNA extracted from the patients' peripheral blood mononuclear cells. The overall sequence similarity between the Korean KCCH and Japanese prototype ATK strains of HTLV-I were 99.1% and 99.1% based on 680 nucleotides and on 226 amino acids, respectively. Phylogenetic analysis, using the maximum parsimony method, showed that the Korean HTLV-I strain grouped with the cosmopolitan HTLV-I topotypes. Our data are consistent with the same evolutionary lineage of HTLV-I from Korea and Japan.
