Public health practice comprises organized efforts to improve the health of communities. Public health differs from clinical medicine by targeting populations rather than to individuals, emphasizing prevention and keying interventions to multiple social and environmental determinants of disease.
In 1988 IOM published the pivotal report, The Future of Public Health. The IOM report provided a new categorization of public health functions, which has become widely adopted. Functions were denoted as assessment, policy development, and assurance. In 1994, the US Public Health Service(Public Health Functions Steering Committee) issued a vision and mission statement of public health and listed 10 essential public health services. WHO and other countries, Canada, Australia and UK employed essential public health functions as a tool of strengthening of public health system.
We reviewed the concept, principles and services of public health, investigated the infrastructure of US, Japan and Korea public health system to identify areas of change and actions in seeking to address the challenges facing Korea health system.
① Public health organizations must adopt a population health approach considering environmental, social and behavioral aspects of public health rather than focus individual treatment.
② Public health organizations must support and promote intersectoral partnerships and community networks rather than use command-and-control approach.
③ The structure of public health organizations must be changed to meet the internal and external need of public health. The recommended organization structure is network model.
④ Performance standards based on core functions and essential services of public health must be established and used to self-assessment and surveillance of public health organization’s capacity.
⑤ Core competencies for public health workers must be developed and used as the guide to job descriptions, orientation, training activities and self-assessment.
