The RV144 Thai HIV Vaccine trial showed efficacy of 31% at 42 months; vaccine efficacy peaked at 60% at month 12. Analysis of immune correlates of risk showed that IgG against 1st and 2d variable loops (V1V2) of gp120 envelope of HIV (Env) was inversely correlated with risk of HIV infection, and IgA antibody against Env was directly correlated with risk. In addition, single genome analysis of breakthrough infections has suggested a viral sieve effect in V2, at a residue (K169) that is in contact with monoclonal antibodies from RV144 vaccine recipients. Exciting new data show that further boosting of RV144 vaccine recipients is associated with significant elongation of the HCDR3 region of HIV specific antibody and with improved magnitude and durability of V1V2 responses. A new trial of ALVAC-HIV + bivalent gp120 B/E is planned in men who have sex with men (MSM) in Thailand. Against this background, new studies in early acute HIV infection raise additional considerations. Single genome analysis of HIV from the earliest timepoints post infection show that the majority of infections, as predicted, are the result of infection with a single transmitted founder. However, next generation deep sequencing suggests undescribed phenomenon we have termed “cryptic” infection, where a very small (~1–2%) ... 전체 초록 보기
