Since the hypothesis suggesting vascular depression to be a subtype of late-life depression was formulated in the late 1990s, many studies have been conducted to prove the hypothesis. However, controversies surrounding the definition and concept of vascular depression have made it difficult to integrate and apply the knowledge gained from previous studies. The diagnostic validity of vascular depression has not been firmly established ; nevertheless, many researchers argue that the clinical value of vascular depression should be assessed and emphasize the need for further investigation. In this article, the author explores various issues about the concepts, mechanisms, treatments, and prognosis of vascular depression by reviewing many critical papers discussing the vascular depression hypothesis. If the vascular depression hypothesis is substantiated through further research, it will have important clinical implications in understanding the pathogenesis, treatment, and prevention of late-life depression.
