A study was undertaken to determine the age distribution of positivity of HBsAg and anti-HBs in the infants and children aging from one month to 15 years and living in different areas in Korea. From October 1985 to December 1986, sera of 1,964 cases of children who were ambulatory or admitted to Seoul National University Childrens Hospital were tested for HBsAg and anti-HBs either by enzyme-linked immunosorbent assay or by radioimmunoassy. In addition, for the children whose sera proved to be HBsAg-postive the enzyme activities of the serum alanine transaminase(ALT) and aspartate transaminase(AST) were also measured. The resulsts were summarized as follows : 1. The positive rates of HBsAg and anti-HBs in childrens sera were 4.9%(95/1, 964) and 37.1% (276/745), respectively. 2. The postive rates of HBsAg in male and female children were 5.9%(71/1, 203) and 3.2% (24/761), respectively. Statistically, there was no significant difference between male and female children. 3. The HBsAg could not be detected in the sera of infants who were under 7 months of age, but was detectable in the sera above 8 months of age. The highest age positivity was observed in the group of 11-15 years(6.5%). Similar positivities of HBsAg were, however, found among the age groups : 8 months(4.8%), 11 months(2.9%), 12 months(3.2%), 1-2 years(3.4%), 3-4 years(4.1%) and 5-10 years(5.7%). 4. The highest positive rate of HBsAg was found in children living in Seoul(5.8%), and followed in order by Jeonnam(4.8%), Kyunggi(4.2%), Kyungbuk(3.8%), Chungnam(3.6%), Cheju (3.4%), Jeonbuk and Pusan (each 3.1%), Kangwon (2.3%), Chungbuk (2.0%) and Kyungnam(1.8%). 5. Of 82 cases of children who had serologic marker of HBsAg in their sera, 42(51.2%), and 55(67.0%) showed normal serum levels of AST(< 25 IU/L) and ALT(< 29 IU/L), but 40(48.7%) and 27(32.9%) showed increased enzyme activities of AST(25 to 500 IU/L) and ALT(29 to 500 IU/L), respectively. This study indicated that the Korean children, particularly living in urban areas, attained high positivity of HBsAg in the first 8 months of life, and therefore babies should be vaccinated against HBV infection during early months of birth.
