Biodegradable polymers have been extensively used in biomedical applications, including regenerative medicine and medical devices. However, the ensuing acidic byproducts resulting from degradation can generate vigorous inflammatory reactions, often leading to clinical failure. Here we present a novel approach to prevent acid-induced inflammatory responses associated with biodegradable polymers, such as PLGA and PLLA, by using modified nanoparticles of magnesium hydroxide that neutralize the acidic environment. The magnesium hydroxide embedded in biodegradable polymers was employed for in vitro chondrogenic differentiation from human mesenchymal stem cells (MSCs). As a result, magnesium hydroxide embedded in biodegradable polymers inhibited the inflammatory cytokine production and enhanced further differentiation of MSCs compared to control polymers. In particular, we demonstrated that the modified magnesium hydroxide embedded in implanted biodegradable polymers remarkably alleviates tissue inflammatory response in subcutaneously implanted rats and chondrogenesis in vivo. We also observed significantly reduced inflammation and stenosis in porcine coronary after stent implantation. Finally, we demonstrated that decreased inflammation allows better reconstruction of mouse renal glomeruli in a tissue regeneration model. Such a modified magnesium hydroxide can be used to extend the applicability of biodegradable polymers to various biomedical engineering fields and to improve clinical success rate in all therapeutic areas using biodegradable polymers.
