Background : Cerebral infarction is one of the most common causes of death in Korea. However, there are few epidemiologic studies about mortality and recurrence after first cerebral infarction in Korea. This study was designed to investigate the mortality and recurrence after first cerebral infarction in Chungju area. Methods : A cohort of 257 patients with first-ever cerebral infarction at our department were selected and followed for at least 5 years (30 days, 1 year and 5 years). Cumulative mortality and recurrence were calculated with the use of a Cox proportional hazards model. Results : The cumulative risk of mortality after first cerebral infarction was 13% at 30 days, 21% at 1 year, 49% at 5 years. The cumulative risk of recurrence after first cerebral infarction was 4% at 30 days, 11% at 1 year, 24% at 5 years. Significant risk factors of mortality were age(≥65yr), heavy alcohol drinking history, heart failure, untreated diabetes mellitus, untreated hypertension, high hematocrit(≥50%) at admission, high blood glucose(≥200mg/dl) at admission, atrial fibrillation at admission, abnormal mental state at admission, embolic infarction type, high systolic blood pressure(≥160mmHg) at admission, high diastolic blood pressure ≥100mmHg) at admission. Significant risk factors of recurrence were age(<65yr), male sex, heavy alcohol drinking history, ischemic heart disease, untreated diabetes mellitus, high blood glucose(≥200mg/dl) at admission. Conclusions : These data from Chungju area may not represent those of our country, but diabetes mellitus, hypertension, heavy alcohol drinking, cardiovascular disease are considered to be important factors of the prognosis of cerebral infarction patients.
