폐 종괴를 처음 발견하였을 때는 예후와 치료 방법이 달라질 수 있기 때문에 원발성 종양에 대한 고려, 영상학적 소견, 임상소견을 충분히 고려하여 원발성 폐암인지, 전이성 폐암인지에 대한 구별이 필요하다. 저자들은 신세포암으로 우측 신장 절제술을 받은 62세 남자가 특이 증상 없는 컴퓨터단층촬영 상의 양측 각각 26 mm, 4 mm 크기의 종괴로 내원하여 시행한 조직 검사에서 우측 폐에서 원발성 편평상피세포암 진단 후 우측 우상측 폐엽 절제술 시행 및 그 후 좌상측 폐 종괴의 조직검사 통해 신세포암의 전이가 발견된 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.
