Accurate diagnosis for substance abuse is very important for treatment and intervention. This research examined the diagnostic accuracy of the three MMPI-2 substance use/abuse related scales (AAS, APS, and MAC-R) by comparing substance abuser group and psychiatric inpatients. In terms of substance abuse diagnosis, AAS was the most accurate measurement scale than APS or MAC-R. Optimal cutoffs for AAS, APS, and MAC-R were calculated. The cutoffs for male substance abuser group and male inpatients for AAS, APS, and MAC-R were T≥60, T≥54, and raw score≥24, respectively. The male substance abusers who qualified above all three cutoffs for the three scales also had a higher SCL- 90-R, most of the MMPI-2 clinical, content, and supplementary scales. In contrast, those who qualified below all three cutoffs showed higher scores on L and K validity scales whereas other clinical scales were significantly low scored. This is indicative that those who scored low on AAS, APS, and MAC-R tend to be defensive and display vigilant personality features. For those who score high on AAS, APS, and MAC-R are likely to admit their substance related issues and are motivated to seek treatment. This also suggests that substance abuse treatment and intervention should take into account accurate diagnosis along with psychological, personality related, and clinical features.
