Alopecia neoplastica is defined as hair loss secondary to avisceral malignancy that has metastasized to the scalp. The scalpis a relatively common site of cutaneous metastasis, usuallypresenting as a single or multiple firm scalp nodules. Alopecianeoplastica is a well-recognized but rare presentation, and itspathogenesis is incompletely understood. Atrophy of the hairfollicles due to tumor invasion of the scalp plays a role in thedevelopment of alopecia. Herein, we describe a 33-year-oldwoman with gastric adenocarcinoma who developed alopecianeoplastica while receiving cancer chemotherapy. Scalp biopsyrevealed metastatic adenocarcinoma cells interspersed betweencollagen bundles and around hair follicles. Immunohistochemicalanalysis indicated that the tumor cells originated fromthe primary gastric adenocarcinoma. Therefore, she wasdiagnosed with alopecia neoplastica due to gastric adenocarcinoma. The findings from this report may be helpful for understandingthe mechanism of alopecia neoplastica.
