목 적 :
폐 결절이나 종괴에 대한 Percutaneous Transthoracic Needle Biopsy(PTNB) 시술 시 Cone Beam Computed Tomography(CBCT)의 Virtual Navigation(VN)을 단독으로 이용했을 때와 VN와 segmentation(SGT)과 3D polyline(PL)을 함께 이용했을 때 시술의 성공률 및 정확도를 비교, 분석하여 유용성을 알아보고자 하였다.
연구방법 :
폐 결절 또는 종괴가 있어 PTNB를 시행한 환자 226명(남:여=156:70명, 평균나이:68.6±12.2세)을 대상으로 이중 CBCT VN만을 이용한 환자 55/226명을 A그룹, CBCT VN과 SGT & 3D PL을 함께 이용한 환자 171/226명을 B그룹이라 정하였다. 그리고 두 그룹의 진단적 성공률, 민감도, 특이도, 정확도를 비교하였으며 시술에 의한 합병증 발생 분포를 비교 분석하였다.
결과 :
1) 정량적 평가에서 조영제 희석 비율 전체의 HU평균 변동률은 proximal-distal RHA 79.5%, proximal-distal LHA 81%로 감소하였으며, RHA와 LHA의 변동률 차이는 조영제 희석 비율이 40% 일때 5% 이상으로 통계적으로 유의하였고, 나머지에서는 차이가 없었다. 2) 정성적 평가 결과, 조영제 희석 비율 각 각의 영상 평가 평균점수는 7.7, 8.58, 8.64, 8.75점으로 희석 비율 40% 일 때 가장 낮은 평가를 받았다. 그중 3 지점의 조영 정도 평가는 distal-RHA, distal-LHA은 희석 비율 100%일 때, HCC feeding artery는 희석 비율 60%일 때 각각 8.8, 8.77, 8.95점으로 가장 우수하였으며, cone-beam artifact 평가에서는 희석 비율 100%일 때 7.8점으로 가장 많이 발생하였다.
결론 :
진단적 성공률 비교 결과 B그룹이 94.7%로 A그룹(90.9%)에 비해 더 높았고 민감도(sensitivity), 특이도(specificity), 정확도(accuracy) 비교에서는 A그룹은 79.1%. 100%, 83.6% B그룹은 88.5%, 100%, 91.8%로 B 그룹의 진단적 특징이 더 우수한 것으로 나타났다. 두 그룹의 시술 합병증 발생 비교 결과 B그룹이 시술 합병증을 53.2% 줄일 수 있었다.
