Purpose : To understand the visual recognition responses in the anisometropia and monovision, we investigated event related potentials (ERP) changes in the visual cortex. Methods : Fifteen young adults (23.00±1.60 yrs) were enrolled and exclusion criteria were included corrected visual acuity worse than 0.00 logMAR in each eyes, myopia < -2.00 D, astigmatism > -0.75 D, anisometropia > 0.50 D, abnormal accommodative amplitude, and any history of refractive or other ocular surgery. Anisometropia was induced in the non-dominant eye with contact lenses of additional +1.00 and +2.00 D power over the refractive power. The facial expressions of neutral and sad were displayed on the monitor as stimuli and ERP was recorded using NeuroScan SynAmps 2. The amplitudes and latencies at P100 and N170 associated with visual recognition in the induced anisometropia state were calculated and compared. Results : Monocularly, in 1.00 and 2.00 D induced anisometropic eyes, the P100 amplitudes associated with the initial visual response were stronger and the N170 amplitudes associated with recognition response were weaker than dominant eyes (p=0.010, p=0.040, respectively). Binocular vision in the 1.00 and 2.00 D induced anisometropic eyes, the N170 amplitudes were weaker and the latencies of N170 were longer significantly compared with isometropia (p=0.010, p=0.010, respectively). Conclusion : In the induced anisometropic non-dominant eye, the initial visual response was stronger than dominant eye. In the binocular vision of aniosmetropic condition, there was no difference in the initial visual stimuli, however, in the visual recognition process, the visual stimulus was weaker and the time for recognition was longer compared to the isometropic state. This study is expected to provide information on the visual perception of anisometropia and help to understand the visual response of monovision.
