Hepatic portal vein gas has known for a life-threatening radiologic sign associated with various underlying abdominal diseases. In neonates, air in the portal venous system is a rare finding. It is mainly observed in infants with progressed necrotizing enterocolitis, which may require immediate surgical intervention, so we need a close observation in that case. We report a rare case of a preterm neonate with portal vein gas due to malpositioning of the umbilical catheter other than necrotizing enteritis. Portal vein gas was detected after the umbilical venous catheter was misdirected into the portal vein, and 6 hours later, it disappeared. This case reports on transient portal vein gas caused by complication of umbilical vein catheterization, and is intended to help determine the direction of examination and treatment when air in the portal vein occurs in neonates.
