Kabuki syndrome is a rare congenital disorder characterized by distinctive craniofacial and dental abnormalities, including tooth agenesis, eruption disturbances, cleft palate, and a high-arched palate. These features present clinical challenges for orthodontic management during growth. This case report describes the dental management of two pediatric patients with Kabuki syndrome, with a particular focus on orthodontic considerations. In the first case, a 9-year-old boy presented with congenitally missing teeth. Clinical and radiographic evaluation revealed multiple missing teeth and ectopic eruption of a maxillary canine. Treatment was planned to guide the eruption path of the canine while preserving future treatment options during growth. Aggressive orthodontic intervention was intentionally avoided in view of the patient’s mixed dentition and underlying syndromic condition. In the second case, a 6-year-old boy presented with congenitally missing teeth and ectopic eruption of permanent teeth. Clinical and radiographic evaluation showed multiple missing teeth, ectopic eruption of the maxillary first permanent molars, and a constricted maxillary arch. Based on these findings, orthodontic treatment was initiated using a rapid palatal expander and facemask for the management of a skeletal Class III relationship. However, limited patient cooperation led to treatment-related difficulties and temporary discontinuation of active orthodontic treatment. These cases suggest that orthodontic management in patients with Kabuki syndrome should take into account not only anatomical findings but also patient cooperation, behavioral characteristics, and developmental status. A staged and flexible treatment approach coordinated through pediatric dentistry may facilitate individualized clinical decision-making while preserving long-term treatment options.
