To examine the effects of a 12-week Reformer Pilates program on body composition, chest circumference, and postpartum recovery in third-trimester pregnant women. Twenty women were randomized (exercise/control, n=10 each) using a random number table; the exercise group performed Reformer Pilates three times per week (individual intervention period: 10–14 weeks, depending on delivery date). Lower-extremity strength (30s-CST) improved significantly in the exercise group (14.3±3.7 to 23.1±2.6 reps, p<.001); and the improvement was greater than in the control group (between-group t=5.72, p<.001). Skeletal muscle mass tended to increase in the exercise group but decreased in the control group. In a small cesarean subsample (n=5 vs. n=7) time to first ambulation was shorter the exercise group(18.2 vs. 27.6h, p=.003). Chest circumference differed between groups only after adjustment for baseline values (ANCOVA, p=.009), and pregnancy stress showed no significant change. Reformer Pilates may improve lower-extremity function in late pregnancy, and the shorter postpartum ambulation time and changes in chest circumference should be interpreted as preliminary findings requiring confirmation in larger trials.
