The purpose of this study was to systematically review the characteristics of exercise interventions for middle-aged and older adults with developmental disabilities and to examine their overall effects through meta-analysis. Considering the premature aging characteristics of this population, studies with a mean participant age of 40 years or older were included. Following the PRISMA 2020 guidelines, studies published between 2016 and 2025 were systematically searched and screened. A total of 18 studies were included in the systematic review, of which eight independent study effects met the criteria for quantitative synthesis. Hedges’ g was calculated using a random-effects model, and sensitivity analyses were conducted to examine the robustness of the pooled effect. The overall effect of exercise interventions was statistically significant (g=.417, 95% CI [.078, .757], p=.016), indicating a positive effect compared with control conditions. The pooled effect for physical function was also significant, although substantial heterogeneity and sensitivity to an influential study were observed. The direction of the overall effect remained generally consistent across sensitivity analyses involving pre-post correlation assumptions, cluster-design adjustments, and alternative approaches to multi-arm studies. However, the limited number of studies, methodological heterogeneity, and risk of bias warrant cautious interpretation. These findings suggest that exercise may serve as a meaningful non-pharmacological strategy for maintaining health and functional capacity in middle-aged and older adults with developmental disabilities. Future research should focus on well-designed randomized controlled trials and individualized adapted exercise programs that address functional decline, sustained physical activity participation, and aging-related needs.
