    비스테로이드성 항염증제(NSAID)는 가장 일반적으로 사용되는 소염?진통 약물이지만 위장관 질환, 심혈관 질환 및 간독성 등 다양한 부작용을 유발하여 일부 약물은 시장에서 철회되었다. Drug-Induced Liver Injury Network에 수집된 자료에 따르면 Diclofenac이 조사된 NSAID 중 가장 간 손상과 관련이 있는 것으로 보고되었다. Diclofenac의 간독성 기전은 사람 및 랫드의 간세포에서 미토콘드리아의 기능 손상과 활성산소 생성에 따른 산화적 스트레스에 의해 apoptosis가 유발되고 산화적 인산화 및 ATP 생합성을 억제하는 것으로 알려졌지만 정확한 독성 기전은 밝혀지지 않았다. 본 연구에서는 Diclofenac을 포함한 다수의 NSAID가 간세포에서 오토파지를 억제하였으며 대표적인 간독성 약물인 Diclofenac을 이용하여 오토파지와 간독성의 연관성에 관한 실험을 진행했다. Diclofenac은 간세포에서 활성산소를 유발하였고 이는 lysosome의 pH 증가 및 cathepsin의 활성을 저해시켰으며 lysosome과 autophagosome의 융합을 억제했다. Diclofenac에 의해 억제된 lysosome의 기능 및 오토파지를 lysosome 활성 약물인 clioquinol을 처리하거나 transcription factor EB를 과발현시켜 lysosome의 기능과 오토파지를 회복시켰으며, 항산화제 및 항산화 효소 과발현 실험을 이용해 Diclofenac에 의한 오토파지 기능 손상이 개선되었고 이를 통해 NSAID에 의한 오토파지 억제가 ROS 생성과 관련됨을 알 수 있다. 미토콘드리아를 선택적으로 분해하는 오토파지 기전인 미토파지 또한 Diclofenac에 의해 억제되었다. 억제된 미토파지는 활성산소를 소거하거나 lysosome을 재활성시켰을 때 회복되었고 미토콘드리아의 기능 또한 개선되었다. 대표적인 오토파지 활성약물인 rapamycin을 마우스에 투여했을 때 Diclofenac에 의해 유발된 간독성이 회복되었으며 LC3, p62 등의 오토파지 마커를 통해 Diclofenac에 의해 억제된 오토파지가 개선된 것을 확인했다. 결과적으로 Diclofenac은 ROS 생성을 유발하고 생성된 ROS를 매개로 한 lysosome의 기능 손상이 오토파지의 기능 억제를 유도한다. 억제된 오토파지는 미토콘드리아의 기능을 유지하지 못하게 하여 활성산소를 과도하게 생성한다. 이러한 결과들은 NSAID에 의한 간독성이 활성산소와 오토파지를 매개로 유발될 수 있음을 보여준다.
