현대 사회의 노인 인구가 점점 증가하면서 우리나라는 초고령화 사회로 들어서고 있다. 이에 따라 노인성 음성장애의 발병률 또한 높아지고 있다.
기존에 부드러운 성대의 접촉을 유도하는 문맥을 활용한 치료는 많지만, 상대적으로 성대의 강한 접촉을 유도하는 적절한 음소들을 활용한 문맥치료에 대한 연구는 미비한 편이다.
본 연구는 성대접촉 유도 문맥치료 프로그램이 노인성 음성장애 환자의 음성 개선에 미치는 영향을 알아보고자 하였다.
연구대상은 성문의 틈이 관찰된 노인성 음성장애로 진단된 2명의 남성을 대상으로 실시하였다. 치료는 주 2회 실시되었으며, 총 10회기가 진행되었다. 음향학적 평가와 공기역학적 평가는 사전·사후 각 3회씩 실시되었다. 주관적 평가와 후두내시경 검사는 사전·사후 각 1회씩 실시되었다. 또한, 치료 전과 후의 음성 개선 정도를 파악하기 위해 음향학적 평가(Jitter, Shimmer, NHR, CPP, L/H ratio, CSID), 공기역학적 평가(PHOT, MEAF), 주관적 평가(VHI)를 실시하여 치료 전, 후의 음성개선 정도를 살펴보았다.
연구의 결과는 다음과 같다.
첫째, 음향학적 평가 결과, Jitter, Shimmer, NHR, CPP, L/H ratio, CSID가 개선되었다. 둘째, 공기역학적 평가 결과, PHOT와 MEAF가 개선되었다. 셋째, 주관적 음성평가 결과, VHI의 점수가 감소하였다. 넷째, 후두내시경 검사 결과, 성문의 틈이 줄어든 것을 확인하였다.
본 연구를 통해, 성대의 강한 접촉을 유도하는 성대접촉 유도 문맥치료 프로그램이 노인성 음성장애 환자의 음성개선과 성문틈 개선에 효과적인 것으로 확인되었다.
