This study investigated the effect of physical literacy (PL) on health-related quality of life (EQ-5D) and the mediating role of Senior Fitness Test (SFT) components among 63 older women meeting WHO physical activity guidelines. Measurement tools included the Senior Perceived Physical Literacy Instrument (SPPLI), the EQ-5D-3L Index, and the six SFT items. Data were analyzed using PROCESS Macro Model 4 (5,000 bootstrap resamples) while controlling for age, BMI, and chronic diseases. The direct effect of PL on EQ-5D was not statistically significant after controlling for covariates (B = -0.027, p > .05). However, among the six SFT components, upper body flexibility (back scratch) fully mediated the relationship between PL and EQ-5D (indirect effect B = 0.0084, 95% Boot CI [-0.0310, -0.0010]). No significant mediating effects were observed for lower/upper body strength, aerobic endurance, lower body flexibility, and agility/balance. These findings indicate that among physically active older women, PL enhances health-related quality of life specifically when translated into intentional maintenance of upper body flexibility and shoulder mobility, which supports functional independence and pain reduction. Therefore, tailored PL-based exercise interventions targeting upper body flexibility should be developed to optimize quality of life in older women.
