The Assessment that a human being experiences a depressive state is made on psychical grounds. Whatever bodily manifestations may accompany the mental state, such as a loss of appetite, loss of weight, loss of sleep, loss of energy and other manifestations, they are clearly possible consequences of a emotional state. The underlying causes of these bodily manifestations could as well be physical. However, it is a remarkable fact that a large number of psychiatrists not only assume that depression is primarily an organic condition and that the content of depressive thoughts is irrelevant, but consider that the onus of proof that it is not so entirely a matter for the psychically minded. In fact, it is the reverse which is true. The diagnosis of depression is made on psychical grounds and the organicists postulate a causal underlying physical substrate. The psychoanalytically minded in no way deny this physical substrate. They observe the psychical picture, they follow its evolution, and they investigate the possibility of psychoanalytic methods of influencing the typical, repetitive, prototypical mental processes always present in depression.
