Prevention of human errors in industrial systems has been one of the motivational themes to the human factors engineering (HFE) and ergonomics. From the early era of HFE, human error is the main target to overcome if the safety of workers should be verified. In spite of the relatively long list of many fruitful techniques to identify, analyze, evaluate, and cope with human errors, there still arise various new issues and types of investigation techniques. Recently abundant amount of information about human errors and their cases has been accumulated in each industrial field; however, countermeasures proposed are not verified as effective and even related to the human error in real. This paper includes a study on the re-specification for the effective management of human errors, through the comparisons of the safety characteristics of human involvements in each industrial system and the techniques to manage the human error information. It is relatively amenable to conclude to the human error as a cause of an accident, because every accident must have some kinds of human involvements within the event sequences. The meaning of the human error term and the intention of the technical description might be extremely different according to the technical fields. Additionally the validity of human error statistics might become doubtful due to the reduced trend of human error cases in practice. Types of classification schemes and taxonomy for the causes and countermeasures are reviewed based on the experiences in mainly high-reliability industries such as process and transportation fields. A procedural specification proposed as a result in this paper is beneficial to the human error management in practice.
