Breast cancer is a systemic disease. The treatment of breast cancer has become clearer since the knowledge of molecular subtypes. Different subtypes has different prognosis and respond differently to neoadjuvant chemotherapy. 50% of Her2 positive breast cancer will achieve pathological complete response after neoadjuvant chemotherapy plus target therapy; while only 2 % of Luminal A breast cancer can achieve pCR. Katherine is the trial to see whether we should do differently in patients with residual cancer after neoadjuvant therapy for HER2 positive breast cancer. Adjuvant T-DM1 demonstrated both a statistically significant and clinically meaningful improvement in IDFS compared with trastuzumab. The KATHERINE data will likely form a new standard of care in this population and increase the use of neoadjuvant therapy in HER2-positive EBC.
As regards to Luminal A cancer, its prognosis is the best and probably won’t benefit much from chemotherapy. TailorX study is a prospective randomized trial.
