목적: 견봉 하 국소주사 요법으로 ketorolac과 corticosteroid의 효과를 비교 분석하고자 하였다.
대상 및 방법: 견관절 충돌 증후군으로 진단된 환자 20명에게 ketorolac 60 mg을, 또 다른 20명에게 triamcinolone 40 mg을 주사하고, 시간에 따른 시각적 통증 점수(visual analogue scale, VAS), 관절 운동 범위(range of motion, ROM) 및 Constant-Murley score를 측정하여 후향적인 방법으로 비교, 분석하였다.
결과: VAS는 주사 4주 후, 두 군 모두에서 감소된 소견을 보였으나 corticosteroid군에서 ketorolac군에 비하여 유의하게 더 감소된 소견을 보였고(p=0.001), 12주 후에는 두 군 간의 차이가 없었다(p=0.707). Constant-Murley score는 두 군 모두에서 향상되었으며 유의한 차이가 없었다(p=0.677). ROM 역시 두 군 모두 향상되었으며, 외회전의 경우 ketorolac군이 corticosteroid군보다 유의하게 향상되었다(p=0.005).
결론: 당뇨 등의 기저 질환이나 반복되는 주사 치료로 인한 부작용이 우려될 경우, ketorolac은 견봉 하 스테로이드 국소주사 치료에 대한 대안이 될 수 있을 것으로 생각된다.
