배경: C-reactive protein (CRP)과 procalcitonin (PCT)은 감염증 및 염증성 질환의 진단과 경과 관찰에 있어 우수한 지표로 알려져 있다. 본 연구에서는 최근 국내 회사에 의해 개발된 ichroma SMART (Boditech Med Inc., Korea) 장비를 이용하여 CRP, PCT 두 항목에 대한 분석능을 평가하고자 하였다.
방법: ichroma SMART를 이용한 CRP와 PCT 측정결과를 CLSI 지침에 따라 정밀도, 직선성, 상관성을 평가하였다. 상관성 평가에서 CRP는 DxC 800 (Beckman Coulter, USA)의 검사결과와 PCT 검사는 VIDAS (bioMerieux SA, France)의 검사결과와 ichroma SMART 검사결과를 비교하였다. 또한 검체 간 잔효를 함께 평가하였다.
결과: 정밀도 평가에서 총 정밀도 변이계수가 저농도, 중간농도와 고농도(level 1, 2, 3)에서 CRP는 각각 6.32%, 5.75%, 3.56%였으며, PCT는 각각 8.07%, 6.24%, 6.53%였다. 직선성 검사를 시행하여 회귀분석을 시행한 결과 CRP 검사(측정범위 0.1-336.8 mg/L)는 결정계수(R2) 0.9997였고, PCT 검사(측정범위 0.05-60.91 ng/mL)는 결정계수(R2) 0.9982로 나타났다. ichroma SMART와 DxC 800, VIDAS와의 상관성 비교 결과 상관계수(r)는 CRP 검사는 0.997이었고 PCT 검사는 0.992로 우수한 상관성을 보여 주었다. 검체 간 잔효는 CRP가 0.02%, PCT가 0.04%였다.
결론: ichroma SMART를 이용하여 CRP, PCT 두 항목에 대한 검사 평가 결과 정밀도, 직선성 및 기존 장비와의 상관성이 우수하였다. 상대적으로 신속하고 간편한 장비로서 임상검사에 이용하기 편리한 것으로 판단되었다.
