Extremely low-birth-weight (ELBW) infants, defined as a birth weight < 1,000 g, are at a high risk of congenital lung and heart diseases, and often with several complications. The oral manifestations of ELBW infants are enamel hypoplasia, delayed primary teeth eruption, and abnormal hard palate formation. This report presents the dental treatment of a 6-year-old girl having a history of ELBW. The patient had pulmonary hypertension, severe bronchopulmonary dysplasia, and brain lesion disease. Class II malocclusion was observed, and enamel hypoplasia and severe attrition were detected on the primary molars. In addition, prolonged retention of right mandibular primary incisor, and incipient occlusal caries was found on the permanent first molars. The patient was treated with stainless steel crown restorations on the primary molars and preventative composite resin restoration on the permanent first molars. Because of concern of difficulty in maintaining adequate oxygen saturation and uncooperative behavior related to a brain lesion, treatment was performed under general anesthesia. Maintaining the patient’ s oral health even after treatment under general anesthesia requires intensive preventive care such as periodic topical fluoride application, and diet consultations.
