Purpose: Despite a trend of increasing importance in both direct and indirect government support over the past years, few studies have documented these as determinants of hospital performance. This study aims to examine the impact of direct and indirect subsidies on the medium- and long-term management performance of hospitals. Specifically, the study distinguishes direct subsidies into cash and tangible assets, and calculates indirect subsidies based on the sum of the transfer amount to the reserve for specific purpose projects and the total expenditure on specific purpose projects. The objective is to investigate the effects of direct and indirect subsidies on the current and future performance of hospitals. Research design, data, and methodology: This study utilizes the financial statements of general hospitals from 2016 to the recently disclosed year of 2022 to examine the impact of direct and indirect government subsidies on the current and future performance of hospitals. A multivariate regression analysis was employed to analyze these effects. Results: The main findings are as follows: First, as government subsidies increase, the current performance of hospitals decreases, yet their future performance improves. Second, as the portion of exempted taxes increases, both the current and subsequent performance of hospitals improve, but the impact on further future performance is limited. Implications: Most research has focused on a single type of subsidy. This study extends prior research by categorizing direct and indirect subsidies using hospital financial statements and empirically analyzing their impact on accounting performance, thus providing insights into the balanced allocation of subsidies. This study aims to verify the medium- and long-term effects of government-funded projects on accounting performance. This study examines the impact of direct and indirect subsidies on hospital accounting performance over a five-year period. The comprehensive results of this study are expected to offer policy implications for the balanced distribution of direct and indirect subsidies with consideration for long-term outcomes.
