In this paper, I discuss what relation the constructions including a reference form and an address form have with the apposition NP and whether 'refernce form+proper noun' and 'proper noun+reference form' have the same structures or not. Conclusions are as follows. Firsly, when the NPs including reference form and address form are realized as the construction 'reference form/address form(A)+proper noun(B)' or the construction 'proper noun+reference form/address form', the former is the apposition NP construction and the latter is the 'complement+head' construction. Secondly, the two are not in relation of scrambling but basically different with regard to syntactic structure because the former is analyzed as [<SUB>NP</SUB>[<SUB>NP</SUB>A] [<SUB>NP</SUB>B]] and the latter, [<SUB>NP</SUB>[<SUB>NP</SUB>B] A]. Thirdly, all cases are not realized to the reference form or the proper name respectively, but realized to the whole constructions commonly in the two. And I examine various constraints when the reference forms and address forms are used individually and when they are used as heads of proper names.
